Dog License Application Form 

Are you a dog owner?
Dog Licenses are an annual fee and registration is required before March 1st every year, unless the dog came into your care after that date. Any dog older than 3 months (12 weeks) must be licensed.  Two dogs can be registered using this online form.  Maximum 2 dogs per household. Proof of rabies vaccination is required.

Applications will be processed within two to five business days of receipt of payment. 


Registration Rate (Jan 1 to Feb 28):

Altered Dog: $27.56 per dog

Unaltered Dog:  $33.08 per dog

Late Registration Rate (March 1 to December 31):

Altered Dog: $44.10 per dog

Unaltered Dog:  $49.61 per dog


Renew Lifetime Dog Tag: Complete the form to renew your dog's existing dog tag for this year. You will need to provide the number found on their tag. 

Apply for a New Lifetime Dog Tag: If you are applying for a new liftetime dog tag online, your tag will be mailed to the address provide​d​ on the form. 


Guide Dogs and Service Dogs:
Guide and service dogs must be licensed however fees are waived (maximum of 2 dogs total per household).  The Town requires written proof that your dog is recognized as a service dog by a licensed practitioner. See Accessibility in Ontario website for further information: Accessibility in Ontario: what you need to know | ontario.ca.  If you are licensing a service dog please register in person at the municipal office at 575 Dominion Avenue to avoid payment using this online form.  To pre-fill the application it can be accessed on the Town's website under midland.ca/forms.
